Services: Stones River Center is an Inpatient Intermediate Care Facility for people with Intellectual Disabilities located in Murfreesboro, TN.  We are licensed by the State of Tennessee and are in compliance with the Medical standards of the Department of Intellectual and Developmental Disabilities Servies.  The facility employs over 140 staff and provides supports and services to 80 individuals with Intellectual Disabilities.

 Stones River Center is owned by Resource Healthcare of America, Inc.  It is managed under contract by DDMS, LLC of Memphis, TN.  The facility consists of 10, eight person homes located in residential neighborhoods throughout Murfreesboro, TN and a Day Program Site. 

 The homes are staffed 24 hours a day by trained staff.  Each home has a vehicle for its own use.  The homes are set up to purchase groceries, supplies, and clothing in the community.  The individuals that live in these homes assist and participate in these activities.  To the extent possible, the individuals participate in the operation of their home through Family Council meetings, planning their recreational activities and other household duties.  The homes are equipped with 24 hour a day monitored services for fire safety and security.  Homes meet the standards of the National Fire Protection Association with automatic sprinkler systems and fire doors.  

 The Day Program Site serves as a multi-use building.  It houses the administrative offices and the vocational training portion of the program.  Our focus is to provide supports and services that will allow each person to function as independently as possible in their home and community.  

 Professional Staff: Psychiatrist, Primary Care Physician, Nurse, Psychologist, DSP, Occupational Therapist, Physical Therapist, Registered Dietician, Licensed Senior Psychological Examiner, QDDP’s

Criteria of Admission:  The facility shall consider admission for any individual who:  is in need of active treatment or health services; meets the definition of Intellectually Disabled as defined in 42 CFR 435.1009 of the Medicaid program; and has been assessed as appropriate by the Interdisciplinary Team for the active treatment services provided by the individual facility or program.

Exclusionary Criteria: Any individual requiring Long Term Nursing Care (24 hour/ 7 day a week nursing care), any individual that does not have a diagnosis of Intellectual Disability, any individual that would not benefit from participating in active treatment services.
